וביאר התוספות שהשלישי לא יתן כתובה אפילו לפני ששהה עמה עשר שנים כיון שהוי מקח טעות. והרמב"ן מבאר וז"ל "אבל מ"מ צריכה גט שאין כאן טעות ודאי אלא חששא היא וגבי ממון עליה להביא ראיה ולענין איסורא לחומרא וצריכה גט" ע"כ. ומבואר שהחזקה לא עושה אותה לוודאי עקרה ולכן צריכה גט

ויש להוסיף שהסיבה שזה רק חששא כיוון שנוגד לטבע שאשה בטבע כן מולידה אך חזקה שמססיעת לטבע (רוב) כן תהיה בגדר וודאי. כ"ך נראה ועיקר הדברים מצאתי באחרונים שונים

i was wondering about that point, why she should need a get acc. to tosafos since the kiddushin was a mekach ta'os. i thought that perhaps the kiddushin was not a mekach ta'os just that he would of stipulated with her that she does not get a kesubah, so the kesubah is a mekach ta'os.
I think that you are equating mekach ta'os with chazaka, when in fact they are 2 different things:
Tosasfos explains that she gets kesubah from the second husband but cannot marry a third since for money we need 3 cycles of 10 years for chazaka. If so, she should also get the kesubah from her first husband since she was married for less than 10 years? Tosafos answers that although regarding kesubah i.e. money, she doesn't yet have a chazaka, it is mekach ta'os since he can claim that he wouldn't of married her had he known her history. It is not the chazaka that makes her loose kesubah from husband #3 since it is within 10 years, rather it is mekach ta'os. Now, the ramban asks, if he claims mekach taos on kesubah, he should claim it on the original kiddushin as well and she should not need a get? Ramban answers that we aren't convinced by his claim of mekach ta'os. Meaning the mekach ta'os claim is only a safek, not a vadai. BUT, the chazaka after 3 times i.e. she was married to the third husband for 10 years, would be vadai and would make her loose kesubah from the third husband (even if he knew her history when he married her).
B'kitzur, the safek that the ramban suggests is not on the chazaka, it is on the mekach ta'os.
interesting, didn't look at it this way, but we don't have a proof either way what the ramban means...
i have a proof, i think. איילונית tosfos says in 2b doesn't need a גט so this עקרה if we know for sure is an עקרה because of the חזקה should not require a get, you see that it isn't a certainty,
